This just happened a few minutes ago, I posted a reply that was going to start a new page, it says it was posted, but when I tried to look at the post, it showed the page, but not my post which was the only one on the page at the time. It's in the Who are you topic on page 41 to be specific.

Yeah I tried to get to page 41 but it is "blank".

Maybe cuz black d just posted at the very end of the page and the board thought it need more room. So it made another page.

The "Who are you?" topic is a special case: it's affected by a weird bug that I haven't managed to fix yet. It's a leftover from the XMB, where the bug on that topic was even worse. I'm hoping to get it fixed someday, but it's only a minor annoyance so I'm not spending time on that issue right now.
