And the Budokai Tenkaichi battles that you have to "lose" don't count, because they're not scripted to be impossible.

No, I'm pretty sure that the code quite literally does not allow you to win.

Some games have decent enough writers that they can make both scenarios work. I'll take Chrono Cross as an example. Spoilers party!

When you reach the top of Fort Dragonia with your team, Lynx will use some trick to switch Sage's body with his. Shortly after, you enter a battle, as Lynx, against your own team. Ouch. This is a battle that you're supposed to lose (three against one isn't a good start). However, if you're playing with a New Game +, it's entirely possible that you're strong enough to manage to defeat your team. If that happens, the game will move over to a cutscene where Lynx (in Sage's body) launches another attack to take you over anyway.

hardest challenge I have done
1. beat Moloch on the first try on easy in morttal kombat deadly alliance.(Reptile)(complete)
i would have beaten everybody without losing once but i lost to Kitana and Quan Chi.
2. beating ALPHA-152 on easy one round perfect in dead or alive 4.(ayane)(copmlete)
Im not sure how i did that though
3.conquest mode for mortal kombat deadly alliance(complete)
it was hard doing some of the longer combos.
